What to Bring to Your Child's Appointment

Not sure what to bring to your child's appointment? The following checklists will help you plan accordingly.

For your child's first clinic appointment, you will need to bring the following:

  • Copies of any applicable X-rays, lab reports, and medical summaries
  • Results of your child's most recent diagnostic tests
  • If your child has had surgery or other treatment, a treatment summary will be needed

For each of your child's clinic appointments, you will need to bring the following:

  • Referring physician’s name, address, and phone number
  • Pharmacy information for the pharmacy where you would like prescriptions filled
  • Patient identification card  (only if your child has been seen before at UNC)
  • Insurance card and any other related forms required by your insurance company
  • List of allergies
  • Any medications the child is taking
